How do I submit a new vendor application?

We are happy to become a vendor for your organization. Please send your vendor application to info@flyleafpublishing.com and we will return the form(s) to you as soon as possible.

Where can I download your W-9 and Sole Source letter?

How can I change my account settings?

Once you are signed in, navigate to Account Information to:

• Change your email preferences.
• Reset your password.
• View your organization's tax exemption status. If this status is not correct, please contact us.
